Consultant Anaesthetist Permanent position available immediately. Counties Manukau District Health Board provides a quality healthcare service to a large and fast-growing multi-cultural community in the south of New Zealand's largest city. It operates over two sites, Middlemore Hospital, an 860 bed hospital with 11 operating theatres, and Manukau Surgical Centre, a purpose built elective surgical unit of 78 beds and 10 operating theatres with a separately staffed Perioperative Care Unit.

There is a wide range of clinical sub-specialties to tertiary level, including the National Burns Unit, but excluding cardiothoracic and neurosurgery. A variety of terms and conditions of employment can be considered for the right applicant, including flexible hours of work, the option to work on-call or not as desired, and a negotiable start date. A broad scope of tertiary level clinical capabilities, a commitment to teaching and to continuing medical education are essential, as is a strong focus on continuing quality improvement, development and administration of departmental activities are essential. There is a large variety of clinical work, both adult and paediatric; an Acute Pain Service, an Anaesthetic Preadmission Service, a busy Obstetric Unit, and a strong focus on Regional Anaesthesia.

The environment is challenging, but friendly and supportive. There is a heavy acute workload, requiring specialist cover on-site in the evenings and weekend days to support 22 Registrars and SHO's. This is covered by the On-Call System. FANZCA or equivalent is required, and registration with the New Zealand Medical Council is essential. Applications are invited for the position of Acute Medicine Physician. Applicants with additional sub speciality interests are also welcomed. The position is flexible and could suit either a full-time physician with both acute medicine other interests or a part-time candidate whose sole focus is Acute Medicine.

Serving a regional population of 150,000 people, Hawke’s Bay Hospital is one of the largest regional hospitals in New Zealand, with 400 beds and all major specialties represented on-site except Neurosurgery, Plastic Surgery and Cardiothoracic Surgery.

The successful applicant will be the second of two Acute Medicine Physicians responsible for acute management of medical patients in our 20 bed Acute Assessment Unit (AAU). The unit is contiguous to and ably supported by a busy regional Emergency Department (31,000 annual visits with high medical and surgical acuity), a high-spec Radiology Department and a level II Intensive Care Unit. The AAU manages ~2000 medical patients annually with a broad range of acute medical conditions. Average length of stay is 24-30 hours hours with approx. 15% of patients going on to more long term hospitalization. You would be working closely with our team of 14 Consultant Physicians, whose expertise covers all the major subspecialties and 5 Emergency Medicine specialists. This is a great opportunity to provide clinical guidance as part of a strong team of professional staff, who are focussed on providing high level acute care.

The successful applicant must be eligible for vocational (specialist) registration with the Medical Council of New Zealand and have an interest and expertise in the initial assessment and acute management of medical patients.

Consultant Respiratory Physician

We invite applications for a full-time or part-time Consultant Respiratory Physician post.

The post is based at Middlemore Hospital, an accredited, 800-bed, teaching hospital linked with the University of Auckland.

The appointee will join 9 existing respiratory physicians and a large Medicine Department in providing services to a population of around 470,000 people. Middlemore Hospital has a specialist respiratory ward. The respiratory medical team consists of 9 Respiratory Physicians, 2 Registrars, a Research Fellow, a Nurse Practitioner and 5 Nurse Specialists offering all respiratory services except for cystic fibrosis and lung transplantation.

Respiratory services include sleep and oxygen services, pulmonary rehabilitation, acute and chronic non-invasive ventilation, bronchoscopy, thoracoscopy, a fully equipped lung function laboratory, and a regional lung cancer service. We have an interventional bronchoscopy service (stents, valves), and an endobronchial ultrasound (EBUS) service. The department is also active in research and has interests in a variety of areas including asthma, COPD, bronchiectasis, lung cancer and pleural diseases.

The appointee will participate in respiratory ward work, general respiratory and sleep clinics, bronchoscopy and ward referrals. The appointee will also be involved in undergraduate and postgraduate teaching. Dependant on the experience/expertise of the applicants the post may be combined with a small component of internal medicine.

Salary and conditions of service will be in accordance with the New Zealand ASMS Award Agreement. Opportunity exists for attractive salary packaging and private practice.

Applicants must have suitable training and experience in Respiratory Medicine and be able to be registered with the New Zealand Medical Council. Formal training in sleep medicine, research and interventional bronchoscopy are preferred but not essential.

Capital and Coast District Health Board (CCDHB) is currently expanding the delivery of Cardiology Services and as a result are seeking an interventional cardiologist to join the existing 4 interventionalists.

Based in Wellington, New Zealand’s capital city, you will be in the cultural hub of the country. With a population of 240,000, Wellington is situated on a magnificent harbour and framed by hills. It is easy to explore Wellington on foot, and you’re never out of sight of a good coffee! An abundance of excellent cafes and restaurants are within easy walking distance as are galleries, museums and theatres. The city is encircled by a “Greenbelt” which ensure plenty of open spaces to explore by walking and cycling on your days off.

The Cardiology service provides secondary and tertiary inpatient services for the local and greater regional population of 1.1 million people across the lower North Island. Services include a full range of diagnostic, interventional and pacing/ICD services and is currently expanding to include electrophysiology.

The new Wellington Regional Hospital is a state of the art facility which was commissioned fully in early April. It is centrally located in Wellington and is collocated with the Otago Medical School. The new hospital provides Cardiology with a three suite cath lab and a 36 bed cardiac specialty ward which includes general cardiology, CCU and cardiothoracic services.

In addition to the new facility the DHB is also undertaking significant period of capital investment and redesign. This includes recent investment of $1million in new echocardiography equipment and full replacement of cath lab equipment for the new facility.

The existing seven strong Senior Medical team are well supported with experienced junior medical, technical and nursing teams, along with an expanding ambulatory and community based service provided by Rehab and heart failure nurses.

The Service has a large clinical trials and research focus, including 3 research staff and planned introduction of a regional Clinical Research Unit will support the service further.


The package offered to Senior Medical Officers employed by Capital and Coast DHB includes:

This position is fixed for a period of one year due to the ongoing review of the Clinical Director position. There is possibility of the position being made permanent pending the outcome of the review.

This is a pivotal role in ensuring the efficient functioning of the Level 9 operating rooms and the professional practice of the anaesthetists working in that area. Working in partnership with the senior clinical staff of the unit, the Clinical Director will take responsibility for the operational and clinical aspects of the service delivery.

Taranaki Base Hospital, situated in New Plymouth, is a progressive regional secondary referral centre for the province of Taranaki with a wide range of medical specialties, an Intensivist-managed ICU, an HDU and a well equipped Radiology Department providing interventional radiology and respiratory related support, which includes digital radiology, CT guided biopsy, MRI, CT and angiography.

Taranaki Base Hospital has video bronchoscopy facilities and a respiratory lab for spirometry and DLCO testing. A thoracic surgeon is available locally for referral for VATS/lung resections.

The Renal Service is based in New Plymouth, with outreach services provided around the province. The Dialysis Unit is supported by well trained nursing staff. There are links with a tertiary Renal Service.

There is an excellent Laboratory Service and the IT service is among the best is the country. An $80m building redevelopment is due for completion in 2012.

Taranaki is situated on the west coast of the lower North Island of New Zealand with a population of approximately 105,000. It is an area with outstanding scenery and a stable economy. Medicine for the Elderly with its acute medical and rehabilitation components are delivered from a purpose built facility on the Rotorua site. Our new Senior Specialist team is leading exciting changes in Emergency Medicine at Southland Hospital. The role offers diverse experience in a new state-of-the-art-facility and will be instrumental in leading this new era of emergency medicine delivery throughout Southland.

The department supports a rural population of 107,000 and the catchment area stretches from Stewart Island to Te Anau and Queenstown. The open plan design supports 21 beds and a raised central work station, providing visibility to most clinical areas. It includes a de-contamination area, seclusion room and negative pressure room. It also has a five bay ambulance port and is located 50 metres from a helicopter pad. As part of the unique design of the hospital, ED is co-located with the Fracture Clinic and Medical Imaging and in close proximity to Critical Care, Operating Theatres and Security.

The 30,000 ED presentations per annum will be managed by a full team of 3 ED Specialists, 8 Medical Officers and 6 House Officers. We are currently looking for Senior Consultants or Medical Officers to join the team. You will be expected to work a 24 shift pattern. The Medical Officer should have at least 4 years post-graduate experience including 3 years full-time emergency experience. The successful applicants will preferably have achieved EMST/ATLS, ACLS or APLS.

The service is supported by a team of experienced midwives, obstetric, gynaecology and colposcopy nurses.

You will be expected to work an average of 50 hours per week (inclusive of on-call) and participate in a 1 in 4 on call roster. This will include participation in outpatient clinics in Queenstown, Te Anau and Gore. The successful applicant will also be required to undertake regular colposcopy clinics.

Educational development is a priority with daily obstetric tutorials and weekly teaching sessions. The department is accredited for O&G training by RANZCOG and is affiliated to the Dunedin School of Medicine.

You would be the sole respiratory physician and work closely with the tertiary level services in Otago and Canterbury, which are within close geographical location. We have vacancies for Consultant Radiologists at the Auckland City Hospital and Greenlane Clinical Centre.

The Radiology Department is organised into body sections and the current vacancies are in body imaging. You would have an interest in body imaging in all modalities involved, with the ability to perform a range of biopsies and basic interventional procedures. Alternatively, you could have an interest in Emergency Radiology.

The Radiology Department in Auckland has moved to its new facility in the Auckland City Hospital and has developed the Greenlane Clinical Centre as an Outpatient Facility. The department services an active specialist referral base with tertiary regional and national services. There is an AGFA PACS system with a virtually filmless environment.

The Inpatient facility at The Auckland City Hospital has 3 multislice CT scanners, 2 new angiography rooms and plans are underway to upgrade the third room to a combined surgical and interventional radiology room. There is MRI and high quality Ultrasound. The Paediatric Hospital adjacent is run by specialist paediatric radiology staff, some of whom rotate into this department. They have a separate CT but share MR and Interventional/angiography facilities. There is a further sub section of ultrasound servicing the National Women’s Hospital section of the facilities, staffed by Radiologists and Obstetricians.

A satellite Radiology Department adjacent to the Emergency Department and the Admission and Planning Unit has one of the multislice CT scanners, ultrasound and plain film rooms. This area handles most initial investigations of Emergency Department patients and patients for assessment for possible admission. There is very active clinical interaction in this area. A candidate for this position may have an interest in Emergency Department Radiology, and be able to work in this area as their area of interest.

The Outpatient facility has Ultrasound and general radiology with a public/private CT.

This is a big, exciting department, with active clinicians, high quality technical and nursing staff, and is excellently equipped. You would need to be a person who enjoys working in this type of multidisciplinary environment. There are frequent clinical conferences, and the department is the centre for registrar training for the Auckland region. The departments are part of the teaching facility for the University of Auckland and there are academic appointments within the radiologist staff.

Salary will be based on the ASMS collective agreement.

HOURS OF WORK
You will be expected to work up to 40 hours per week rostered shifts inclusive of weekends as per the roster, currently 1:4 ratio. Hours of work may be a minimum of 8 hours per day between the hours of 0800 – 2400.

Although there is currently no call-back requirement it is envisaged that a call-back roster will be developed in the near future following consultation with all affected staff and contractors.

KEY RESPONSIBILITIES

1. CLINICAL PRACTICE
Supports the delivery of quality, evidenced based medical management within contract and budget.

Expected Outcomes:
• Responsibility for overseeing, co-ordinating and personally providing effective, clinically appropriate and safe assessment and treatment, including clinical documentation of patients presenting to the Emergency Department.
• Effective communication, referrals and liaisons with other hospital staff or community medical and nursing staff to arrange ongoing investigation, treatment or care of patients.
• Working with the Head of Department to ensure there is screening of all definitive investigation result reports of Emergency Department patients (within 24 hours of receipt of such reports), correlating with their clinical record and arranging appropriate follow up.

PRIMARY OBJECTIVE(S) OF THE POSITION

1. To provide safe and efficient Pathology and Laboratory services to the Company.

2. To provide advice to the Company on Pathology and Laboratory matters and professional Pathology and Laboratory standards of practice.

3. To instruct, monitor and advise Medical Officers employed by the Company on Pathology and Laboratory principles.

4. To maintain effective interpersonal relationships with colleagues, staff and patients, with the objective of improving hospital services, and handle sensitively any problems which may arise in this respect.

5. To consult and advise Laboratory Technology and Support Staff on all matters pertaining to the function of the Laboratory and its service, both on a routine, or rostered emergency basis. To authorise specific test requirements as necessary.

Note : Where the word 'Hospital' is used this shall be taken to include both Rotorua and Taupo Hospitals and any other site from which relevant Lakeland Health Laboratory or Pathology Services are delivered.

KEY RESPONSIBILITIES

The Pathologist will be responsible for the provision of the following services on a rostered basis with other Consultant Pathologists:

1.1 (a) Anatomical Pathology

The major commitment will be the macroscopic and histological examination and reporting of surgical specimens. The main emphasis is on gynaecological, breast, gastrointestinal and skin pathology.

The Pathologist will be required to participate in the Breast Screening program.

Examination and reporting of frozen sections.

Examination and reporting of gynaecology and general cytological specimens.

Performance, interpretation and reporting of fine needle aspiration cytology.

Performance of autopsies as required by the Company.

(b) Clinical Pathology

The Pathologist will be available to act in a co-ordinating role with regards patient problems in areas of Haematology, Microbiology, Chemical Pathology and Immunology, and refer the problem on to specialist pathologists/consultants.

(c) Locum Cover - Scheduled Leave

The Specialist agrees to provide internal locum cover for their colleague as part of their normal duties where their colleague is absent on a scheduled basis for annual and CME leave.

(d) Locum Cover - Other

Where locum cover is required for any other reason, the Specialist agrees to provide cover on the basis that they receive additional payment in terms of the Employment Contract as scheduled below. In addition, if requested by the Head of Service, after three days of increased roster cover, for reasons other than scheduled leave, the employer, in conjunction with the Head of Service, will immediately try to find external locum cover. If external cover is not available, the employer will negotiate to curtail non-essential services if requested.

1.2 The Pathologist shall notify the Company immediately, through the Head of Service where possible, of any illness which will prevent his / her from performing his duties.

1.3 In the event of a Civil Emergency, the Pathologist will carry out such other duties as may be required from time to time by the Company.

1.4 The Pathologist, in the performance of all of his obligations in respect of this contract, shall maintain an accepted national and international standard and an output at the level fixed by the Company following consultation with the pathologist and his / her colleagues. The Pathologist shall co - operate with the Company in obtaining and collating the data necessary to fix the output levels.

1.5 The Pathologist will see and advise within purchaser contracts on patients referred to him / her by Hospital specialist colleagues, by General Practitioners, midwives and by such other persons as the Company or the Minister of health may from time to time direct. Such advice shall be delivered promptly.

1.6 The Pathologist will provide reports, complete standard Hospital documentation and record data as required and in the manner prescribed from time to time by the Company.

1.7 As appropriate the Pathologist will consult with other health professionals in respect of the care of patients, and will encourage and participate in multidisciplinary team meetings discussing the care and treatment of patients.

1.8 The Pathologist will:

(a) Respect the role of the independent patient advocate
(b) Respect cultural concerns
(c) Respect each individual patient's dignity and rights and ensure that informed consent is obtained in accordance with the Company's policy before undertaking any operation / procedure.

1.9 In order to maintain high standards of practice the Pathologist will participate regularly with colleagues in a review of selected records, case note entries and referral letters.

2.1 The Pathologist will instruct, guide and supervise the work of junior and laboratory staff, give advice as required, and participate in tutorial sessions for all junior resident officers.

3.1 The Pathologist will be familiar with the concept, ethos, structure and purpose of the Clinical Services Model through which the Company operates; or any succeeding model.

3.2 The Pathologist will work, with his / her colleagues and other senior health professionals, and with the Nursing Personnel and other Hospital staff, in the day to day management of the Laboratory and the Hospital generally to ensure that resources are used efficiently.

3.3 As a major user of finite resources, the Pathologist will use resources and procedures in a manner designed to conserve such resources without prejudicing the treatment or well-being of the patient, taking cognisance or information provided by the Employer, advisory groups within the hospital and the Department of Health and other professional organisation as appropriate.

3.4 The Pathologist recognises that the Department must actively participate in the forward planning and budgeting of the Pathology Service to enable the realistic financial allocation to meet the needs of the service to be made.

3.5 The Pathologist recognises that the Department operates under tight budget constraints and will co-operate with the Head of Service and Management to ensure the department operates within budget consistent. Also with the need to safeguard the welfare of patients and will co-operate with the Nurse Leader (or deputy) in respect of Operating Theatre utilisation.

3.6 The Pathologist will be required to participate from time to time in assisting the Company in the formulation of policies and forward plans designed to facilitate the smooth operation and sound management of the Hospital.

Personal Education and Research

4.1 The Pathologist has a key role in the continuing medical education of the Junior Staff, and informing of Senior Medical Staff in respect of relevant clinical matters.

4.2 The Pathologist will maintain currency of knowledge and utilise relevant literature in respect of clinical matters, of medical management topics including clinical budgeting, performance appraisal, quality assurance and medical audit.

4.3 The pathologist will attend conferences on relevant topics both in New Zealand and overseas, and will attend local postgraduate medical meetings; in respect of such meetings and conferences, as appropriate, and as approved by the Company. The Pathologist will report to his / her colleagues and thereby share with them the information and knowledge obtained.

4.4 The Pathologist may undertake research during regular working hours as time permits. Any research undertaken or involvement in therapeutic trials shall first have received the formal approval of the Research Ethical Committee and shall be in accordance with the protocol of that committee. Progress reports will be made to the Bay of Plenty Research Ethical Committee as requested.

4.5 The Pathologist will develop to the best of his ability computer literacy to enable him to utilise modern technology fully.

5.1 The Pathologist, with his / her medical colleagues, will continue the implementation of a quality assurance program (including the updating on a regular basis of protocols and procedures) for the Pathology Service. Assistance may be available from the Company's quality assurance officer (if any) as to the key components of the program. The quality assurance system developed will be regularly carried out and will incorporate review mechanisms in respect of patient care and a monthly audit of death reviews and critical incidents.

5.2 Pathologist Standards of Practice will be consistent with standards set from time to time by the Company (after consultation with the pathologist and with reference to national guidelines).

The Pathologist will be committed to the Company's philosophy of "Continuance Improvement'.

EXPECTED OUTCOMES

The effectiveness with which the Pathologist has performed the job functions set out herein shall be measured with reference to :

(i) The quality of Pathology services provided by the Pathologist taking into account :

(a) The quality of Pathology, the advice offered and the co-operation given to the Company and the colleagues of the Pathologist in achieving the appropriate national or international standards of service for the patients of the Hospital.

(b) Professional skills exhibited in carrying out of Pathology procedures as measured by peer review and quality audits.

(c) The starting and completion of Pathology procedures on and within appropriate time limits.

(d) The number of justified complaints (if any) received by the company in respect of the performance of the Pathologist.

(e) The adherence by the Pathologist to the policy of the Company in relation to patient's rights and cultural sensitivity.

(f) The success of the pathologist in adhering to budget.

(ii) The quality of the relationships developed with colleagues, management and other staff of the Company taking into account :

(a) The level of co-operation between the Pathologist, other pathologists, other senior medical staff, other staff of the Company and management.

(b) Participation by the Pathologist in the formulation of Company plans, strategies and budgets.

(c) Standards achieved by the Pathologist in the completion of Company documentation in an accurate and timely manner.

HOURS OF WORK

Outside Regular Hours

The Pathologist while rostered on call will be available at all times to consult with colleagues by telephone and to be called back to Rotorua Hospital. The Pathologist shall respond immediately and shall attend the Hospital when required as soon as appropriate.

He / she will respond to a paging call as soon as possible.

Regular Hours

Shall fall within 8.00am to 5.00pm - Monday to Friday inclusive
